   Magandang araw po…meron po nanghihingi ng xerox copy ng death cert ng tatay q fr sss dipolog branch. Pra dw po maclose n ung loan balance ng tatay q. Klangan p po b un kung nkapgpasa n aq ng requirements ng funeral claim? At naclaim n po nmin ang funeral cllaim way back 2020 p po. Online application po aq fr QC, Manila. SSS dipolog po bumagsak application q. Pkixplain lng po, salamat po.

   Good day!

   I have concerns po, regarding the SSS form E-1 of my late father, JAIME LUMAPAC LOPEZ (who died last June 7, 2021). There is an erroneous entry, particularly on the spouse’s name of his hard copy of SSS Form E-1.

   Wrong entry spouse name:
   Gelenda Eliot Lopez

   Correct name:
   ERLINDA ELIOT LOPEZ

   We can’t claim funeral benefits due to a discrepancy on his SSS form E-1. These were one of the findings when we applied online for the funeral claim:

   >> WITH DISCREPANCY ON THE DATA IN THE SUBMITTED DOCUMENTARY REQUIREMENTS AGAINST SSS RECORDS

   Pwede ba mi magpa correct sa iyang SSS Form E-1 even he had passed away? Magpa-assist unta ko ma’am/sir, regarding ani nga matter, kung unsay dapat namong buhaton.

   I behalf of my brother application for disability filed last March 2022.
   I would like to follow up the application status, please refer to the details below for information.

   Name of application: Valerio Tubog Martinez

   Address: Natividad Street, Mormons Drive, Tetuan, Zamboanga City.

   He regularly underwent dialysis for twice a week; because of financial constraint, he cannot continue the treatment.

   This program will support, and maintained the treatment schedule required and hoping for recovery.

   I am looking forward to an update at the soonest.
   Please let me know whether it can release this month.
