    I would like to register my father for NSO Certicate to correct my last name for school requirements and His NSO is one of the requirements , he was born in samar since 1964 but he was live in cavite since he was teenager and he was a regular employee until now in private hospital.. he never attend school and no baptismal .. pleas i need your help .. iam graduating next year i have only 5 month to accomplish/correct my last name on my NSO..

    I filed for NSO birth certificate but got a negative record. I was born in 1947 in Tondo, Manila. Perhaps you may have a record ? My complete name is Rolando Manapat Chica.

    I would like to get a certified NSO birth certificate but they said they only issued a birth certificate starting 1945. I believe that my birth certificate was burnt down. I was born on the year 1936. How can i get a certified birth certificate. Need help…..
